体幹筋筋活動の改善が頸部機能に改善を認めた頸部ジストニア患者の一症例  [in Japanese] The Effect on Cervical Function after Improvement of Trunk Muscle Activity in a Patient with Cervical Dystonia  [in Japanese]

今回、頸部左側屈と胸椎右側屈を呈した頸部ジストニア患者(69歳、男性)に対し、その問題点を罹患筋に起因する一次的障害と発症後の代償および廃用によって生じる二次的障害に分類した。それぞれに対して1回の運動療法を実施し、その効果を表面筋電図により検討した。その結果、二次的障害に対する運動療法により体幹筋筋活動が健常者に近づき体幹の偏倚が改善され、それにより頸部の偏倚も改善されたことが確認された。このことは、頭頸部の定位の安定には体幹の定位の安定が重要であるという姿勢制御の観点からも裏付けられると考えられる。

We encountered a patient with cervical dystonia demonstrating left lateral flexion of the cervical muscles, as the primary disturbance (abnormality of the central nervous system) and right flexion of the trunk, as the secondary disturbance (muscle and skin shortening due to disuse). Therapeutic exercise was administered to stretch the trunk muscles and skin, and lateral reaching training in the sitting position was performed for the secondary disturbance. The effect of therapeutic exercise during one session was investigated by clinical signs and surface electromyographic findings. Therapeutic exercise for control of trunk function promoted control of the neck function, especially the righting reaction of the neck during lateral shift in the standing. It was suggested that therapeutic exercise to improve the secondary disturbance is important in patients with cervical dystonia.
