症例は49歳, 女性。数年前より下血があり, 便意も頻回となったため当院受診。直腸指診にて全周性の腫瘍を触知したため即日入院。(1)口唇の色素沈着(2)消化管ポリポーシス(3)家族歴より完全型のPeutz-Jeghers症候群(以下PJS)に発症した直腸癌と診断された。卵巣腫瘍も伴っており, 直腸癌は膣壁にも浸潤していたため腹会陰式直腸切断術+子宮付属器および膣後壁切除術を施行した。術後経過は良好で現在通院加府中である。組織学的には直腸癌, 卵巣癌ともに腺癌であり, 直腸癌の卵巣転移と考えられた。術中に合併切除した小腸ポリープも高分化型腺癌であった。PJSに発症した消化器癌, 重複癌につき文献的に考察した。

A 49-year-old female was admitted to our hospital for symptoms of melena and severe anemia. Digital examination and colonoscopy revealed rectal cancer of the advanced stage. The presence of mucocutaneous pigmentation and diffuse gastrointestinal polyposis with autosomal dominant inherited condition were characteristic of Peutz-Jeghers syndrome (PJS). Advanced rectal cancer with direct invasion to the vagina and metastatic ovarian tumor was observed, abdominoperineal exision of the rectum with hysterectomy and oophorectomy was performed. Intraoperative polypectomy of the small intestines was done to prevent intestinal invagination. Pathological findings revealed multiple primary carcinomas (mucinous adenocarcinoma of the rectum, metastatic mucinous carcinoma of the right ovary and a well-differentiated adenocarcinoma of the small intestines).
