抄録

Uvulo-palato-pharyngo-plasty (UPPP) is the standard surgical technique for treating sleep breathing disorder (SBD). However, clinical improvement has reported to 50 to 65% of the patients. Continuous positive airway pressure (CPAP) has become the standard treatment for treating severe degree SBD recently. However, surgical approach should be reconsidered in the patients who did fit well with CPAP. We reported two patients with severe degree SBD who challenged surgical options. Due to the extensive hyperplasia of palatine tonsil and soft palate, surgical options for severe degree SBD should be done under preparation of tracheotomy. Clinical improvement can be attained if this precaution is strictly followed.
