Abstract
1990年の1年間に皮膚〓痒などの疾病の病原体として北里大学病院に持ち込まれ,北里大学医学部寄生虫学において同定を依頼されたダニの内,興味ある3種類につきその症例とダニの特徴を記載した。症例1は頭頂部に咬着したアカコッコマダニ(Ixodes turdus)雌成虫であり,アカコッコマダニのヒト刺症例として世界で5例目の記録である。症例2は頸部に咬着したキチマダニ(Haemaphysalis flava)雌成虫の例であり,咬着後,四肢の活動性滑膜炎が生じ,ライム病の可能性が疑われたが,血清診断の結果は陰性であった。症例3はムクドリの戸袋への営巣により発生したスズメサシダニ(Dermanyssus hirundinis)雌成虫の例であり,多数の粟粒大の〓痒性皮疹が全身に生じた。近年,自然保護の重要性が認識され,野生の動物が人家近くに住みつくことが多くなり,また自然と親しむことを目的に山野に出かける機会が増えるにつれてダニ刺症の症例も増加の傾向がみられる。ライム病,野兎病,ツツガムシ病などとの関連から今後注目される問題となろう。
In three patients presenting in 1990, mites or ticks were found embedding the scalp or skin, and the vermins were brought to the Department of Parasitology for identification. The arachnids included two varieties of ticks and one variety of mite. A tick removed from the vertex region of the scalp in a female patients 70 years old was identified as the female adults of the Ixodes turdus Nakatsuji 1942. This species of tick is rarely found parasitizing the human body, and to date, the present episode is the fifth case known to be reported in the world. The tick bite in the scalp was characterized by redness and itching. A tick clinging to the neck of 44-year-old woman was identified as an adult female of the Haemaphysalis flava Neumann 1897. The presenting symptoms of the patient gave rise to the suspicion of Lyme disease, an acute inflammatory disease caused by a tick-borne virus. However, the serological test for Lyme disease was negative, and synovitis was disclosed in the left arm and both legs of the patient. In a 25-year-old male, the adult female mite of the Dermanyssus hirundinis Hermann was found. The patient had a rash with a proliferation of popular accompanied by severe pruritus all over the body. The source of the mite was traced to a Starling's bird nest infested with mites and attached to the shutterboxing of the house where the patient was residing. Persons who trek to the mountains and fields become susceptible to parasiteborne infection and zoonotic diseases such as scrub typhus and rabbit fever. General awareness of the environmental possibility of bacterial and viral infection by parasitic transmission is important to prevention and early detection of ailments ranging from annoyance to debilitating disease.
