分娩の曜日・時刻からみた周産期医療の問題点と課題  [in Japanese] Relationship between delivery and days of the week  [in Japanese]

Abstract

札幌市内のA病院で2001年1〜12月までになされた分娩1,008例について,曜日・時刻との関連を検討した。分娩数は,水曜日が最も多く(全体の18.45%),かつ在胎週数34週未満の児と出生体重1,500g未満の児が多く出生していた(それぞれ全体の30,35%)。母児の生命にかかわる胎児心音低下・弛緩出血などの件数は金曜日が最も多く(全体の18.18%),かつ午後14時ころがそのピークであった。また,金曜日には緊急帝王切開(全体の22.77%)・母体搬送(全体の26.09%)が多かった。水曜日に未熟児分娩が集中したのは,以前から入院していたハイリスク児を計画的にマンパワーの手厚い週の半ばに分娩させるためと考えられた。原因の特定には至らなかったが,金曜日に異常分娩が多いことが明らかであった。ハイリスク児出生が同じ日に集中した場合,NICUの負担が過大となる危険性,および土・日曜日に治療・検査を強いられる危険性がそれぞれ危惧された。産科部門・検査部門・NICUの人的資源の配分の際には,このような時間的な要素を考慮すべきこと,地域における関連病院・NICUの相互の連携体制を充実すべきことなどが必要と考えられた。

Purpose: To investigate the relationship between days of the week and live births. Subjects: 1,008 live births during one year (from January 1 to December 31, 2001) at a hospital in Sapporo, Hokkaido. Results: On Wednesdays the percentage of live births was the highest, accounting for 18.45%. 30% of the babies of 34 gestational weeks or younger, and 35% of those with very low birth weight (1,500g or smaller) were born on Wednesdays, also. On Fridays the percentage of high-risk babies born was the highest (18.18%; fetal distress, atonic bleeding, etc.). 22.27% of the emergency Cesarean Sections and 26.09% of the maternal transports were also on Fridays. Discussion: The concentration of the delivery on Wednesdays may result from manipulations to avoid weekends and the beginning of the week for difficult deliveries. The causes for the concentration of the high-risk babies on Fridays were not clear in the present study. But through this concentration on the weekend, large amounts of the work load of neonatal care units are performed on Saturdays and Sundays. When deliveries are planned for a specific day of the week, excessive concentration to one day should be avoided, in consideration of the limits to the facilities of neonatal care units.
