日本の赤ちゃんは出産後に母子同室で過ごせているか : 産婦人科医と助産師を対象とした横断研究より  [in Japanese] Is rooming-in possible for Japanese new-born babies during their first days at institutions? : A cross-sectional study from randomly sampled obstetricians and midwives in Japan  [in Japanese]

Abstract

【背景】本研究では正常分娩直後からの母子同室について,実際にお産を取り扱っている医療従事者の実施状況や考え方を明らかにすることを目的とした。【方法】主たる学会員が産婦人科医および助産師で構成されている団体の会員へ配布公表されている名簿から無作為抽出した474人の産婦人科医と728人の助産師を対象として,自記式質問票を用いた横断研究を実施した。【結果】母子同室の実施に対して「賛成である」と回答したのは,産婦人科医の59.3%,助産所以外に勤務している助産師の88.5%であり,助産所に勤務している助産師においては100%であった。また,対象者がかかわったお産における母子同室の実施割合について0%もしくは20%以下と回答したものは産婦人科医の54.4%,助産所以外に勤務している助産師の56.4%と半数を超えていた。一方,助産所に勤務している助産師において,実施割合が100%と回答したものは95.7%であった。【考察】WHOやUNICEFが母子同室の重要性について提唱し,推進しているにもかかわらず,わが国における母子同室の実施率は十分に高いとはいえない。科学的根拠を重視するならば,早急に病院や診療所の母子同室の実施率を改善する必要がある。

BACKGROUND: In 1996 the World Health Organization introduced the "Care in Normal Birth a practical guide" for promoting evidence based childbirth care. One step suggests that a mother and her newborn baby should remain together day and night during the hospital stay. The purpose of this study was to investigate, first, whether or not mothers in the hospital roomed-in with their babies after delivery in Japan, second, the attitudes of obstetricians and midwives toward rooming-in and their feelings concerning it. METHODS: A cross-sectional study using a structured questionnaire was conducted in 2003 and 2004 among 474 obstetricians and 728 midwives randomly sampled from the member lists of the associations of which most of the members were obstetricians and midwives. RESULTS: All autonomous midwives working for a birthing house (n=24), 88.5% of hospital-based midwives (n=676) and 59.3% of all obstetricians (n=474) were positive toward rooming-in. However, the practice rate of rooming in was between 0〜20% among more than half of the hospital-based obstetricians and midwives. All midwives working for a birthing house practiced rooming-in. CONCLUSIONS: Implementation rate of rooming-in practice recommended by WHO and UNICEF among Japanese hospitals was not satisfactory. It is urgent need to improve perception and practice among health professionals to ensure rooming-in for better initiation of breast feeding for mothers and new-born babies in Japan.
