精神疾患患者の悪性腫瘍に気づくための観察ポイント  [in Japanese] A Study of the Observation Point for Discovery of Malignancies in Psychiatric Patients  [in Japanese]

精神疾患患者の悪性腫瘍に気づいた経験を持つ看護者に調査し,悪性腫瘍発見に繋がる観察ポイントを明らかにすることを目的とした。A,B両県内の精神病院18施設から29ケースのデータを得ることができた。悪性腫瘍名は,胃癌8ケース,大腸癌5ケース,乳癌5ケース,脳腫瘍3ケース,肺癌,膀胱癌各2ケース,陰部癌,睾丸腫瘍,骨肉腫,下顎癌各1ケースであった。それぞれのデータから,気づいた内容を抽出し分析した結果,(1)初めに身体疾患を疑う,(2)広く複合的な観察・確かめ行動を行う,(3)触診を重要視する,(4)漸次的な心身の機能低下を意識する,(5)日常生活への関わり場面を活用する,の5つのポイントを抽出することができた。

The purpose of this study was to explore the observation point for discovering cancers in psychiatric patients. We investigated nurses with the personal experience of discovering malignancies in psychiatric patients. We analyzed data from 29 cases, and observed the following five points; 1) Suspect the body disease in advance of the psychiatric disorder. 2) Do wide composite observation to confirm the assessment. 3) Palpation is an important observation method. 4) Pay attention to the gradual decline of bodily functions. 5) Utilize the opportunity afforded by daily life actions.
