Abstract

目的:大腸癌リンパ節転移に関わる遺伝子を同定することを目的に,CGH法を用いて,手術症例の原発巣について遺伝子解析を行った.対象:2003年4月から2004年12月に当科で手術を行った大腸癌46例を対象とした.このうち,リンパ節転移を伴うものは30例でリンパ節転移を伴わないものは16例であった.方法:手術で得られた大腸癌原発巣の新鮮凍結切片からマイクロダイセクションによりDNAを抽出し,CGH法で解析した.解析結果をリンパ節転移を伴うものと伴わないものとで比較検討した.結果:CGH解析の結果,リンパ節転移を伴わないものでは8p23, 8p22, 16q23において増加を認めた.また,リンパ節転移を伴うものは5p12で増加を認め5q13では欠失を認めた.これらはFisherの直接検定の結果,有意差を認めた(p<0.05).結論:この結果から5p12, 8p23, 8p22, 16q23, 5q13の染色体領域には大腸癌のリンパ節転移に関与する遺伝子が存在する可能性が考えられた.

Objective: We examined the DNA copy number profiles of colorectal cancer (CRC) by comparative genomic hybridization (CGH) in order to identify the gene related to lymph node metastasis. Materials: We reviewed 46 CRC patients who underwent surgical resection at our department between April 2003 and December 2004. Of those patients, 30 patients showed lymph node metastasis and 16 patients were without lymph node metastasis. Method: We extracted chromosomal DNA, by microdissection, from primary carcinomas that had been surgically removed then immediately frozen. We then compared the DNA copy number profiles between two groups. Results: Significant differences (p<0.05) in copy number gains between patients with lymph node metastasis and patients without lymph node metastasis were found at 8p23, 8p22, 16q23 (without>with lymph node metastasis) and 5p12 (with>without lymph node metastasis). Significant difference in copy number losses was found at 5q13 (with>without lymph node metastasis). Conclusions: There is a possibility that a gene related to lymph node metastasis exists in CRC patients and involves 5p12, 8p23, 8p22, 16q23, 5q13.
