• 抗凝固療法中に肺胞出血で発症した急性好酸球性肺炎の1例

Abstract

We report a case of cigarette smoking-induced acute eosinophilic pneumonia with diffuse alveolar hemorrhage. A 53-year-old smoking man, who was under treatment for renal diseases with corticosteroid and antithrombotic regimens, was admitted to our hospital because of pyrexia, bloody sputum and dyspnea. These symptoms occurred on the next day after he had smoked a certain brand of cigarette for the first time ever. Arterial blood gas analysis revealed pronounced hypoxemia and chest CT scans showed diffuse ground glass opacities in both lungs. Bronchoalveolar lavage fluid was macroscopically bloody with microscopic hemosiderin-phagocytosed alveolar macrophages and contained significantly increased eosinophils. The clinical diagnosis made was cigarette smoking-induced acute eosinophilic pneumonia. In addition, anticoagulant therapy for his renal diseases seemed to trigger the development of diffuse alveolar hemorrhage.
