産後うつ病に関連する要因の分析  [in Japanese] Analysis of factors related to postpartum depression  [in Japanese]

Abstract

本研究の目的は,産後1ヵ月の時点の産後うつ病を疑われる母親の割合と関連要因についての現状把握である。研究方法は産後1ヵ月の健康診査に来院した母親を対象に,日本語版エジンバラ産後うつ病自己評価票(以下,EPDS)と日本語版WHO The Subjective Well-being Inventory検査用紙(以下SUBI)を測定用具とした調査研究である。EPDS区分点となる9点以上を「産後うつ病が疑われる」群,8点以下を「正常」群とし比較検討した。その結果は以下の通りである。1.対象者は152名(有効回答率86%)で,そのうち「産後うつ病が疑われる」群は29名(19.1%)であった。2.「産後うつ病が疑われる」群は正常群に比較し,「今回の妊娠出産のための退職」「未婚」「相談相手として夫を選択していない」点で,有意な差が認められた(p<0.05)。3.「産後うつ病が疑われる」群は,母乳栄養の割合が有意に低かった(p<0.05)。4.精神的コントロールに関する褥婦の訴えは,産後うつ病の早期発見として重視すべき情報である。5.産科学的関連要因および新生児学的関連要因に有意な差はなかった。

The objective of the present study was to elucidate the current condition of mothers suspected of having postpartum depression at one month postpartum, in addition to factors related to postpartum depression. The Japanese versions of the Edinburgh Postnatal Depression Scale(EPDS) and the WHO's Subjective Well-being Inventory questionnaire(SUBI) were administered to mothers who visited our hospital for medical examinations at one month postpartum. Mothers with an EPDS score of ≧ 9 points, the cutoff point on the EPDS, were assigned to the "suspected postpartum depression" group, while those with an EPDS score of ≦ 8 points were assigned to the normal group. Comparison of these groups yielded the following results: 1. Among the total of 152 respondents used for analysis in the present study(valid response rate, 86%), 29(19.1%) were classified into the "suspected postpartum depression" group. 2. Significant differences were observed between the normal group and the "suspected postpartum depression" group for the following: "resignation due to the present pregnancy and childbirth", "unmarried", and "selection of individuals other than the husband as conversation partner". 3. No significant differences were observed for obstetric and neonatological related factors. 4. The "suspected postpartum depression" group had a significantly lower proportion of breast-feeding mothers. 5. Complaints regarding psychological control among puerperal women are vital information for early detection and intervention in postpartum depression.
