日本における認知症ケアの人材養成の現状と課題 : 専門研修と専門資格制度の整理から

本稿は、わが国における認知症対策のうち、質的整備の中で最も遅れてきた認知症ケアの人材養成について、1970年代からの認知症対策の歴史的展開を概観しながら、認知症ケアに特化した専門研修制度と専門資格制度について整理し、現状を述べた。その結果、専門研修は階層的な構造を有し、全国的な広がりを見せていることから「認知症ケアの基盤」となっている。また、専門資格制度は、先立って、認定看護師制度が確立しており、続いて学会認定の専門士制度の創設となった。昨年度には、介護福祉士も認定専門資格制度のモデル事業が終了し、近く創設されると予想できることから、これらの資格は、「認知症ケアの牽引役」と考えられる。認知症ケアにおける研修や資格は、ケア現場で協働しながら効率的で効果的なケアの提供に結びつかなければならない。専門研修や専門資格制度をケア現場がどのようにその知識や技術を評価し、活用するかというシステムや職場研修制度(OJT)の構築が未整備である課題が残った。

In the present study, we outlined and described the current state of specialized training and specialized qualification systems for dementia care with regard to staff training for dementia care, an issue that had lagged behind all other qualitative upgrading measures for dementia in Japan, while summarizing historical developments in dementia measures since the 1970s. The results showed that specialized training had a hierarchical structure and had become the "foundation for dementia care" due to its nationwide spread. Regarding specialized qualification systems, the certified nurse system was first established, followed by a specialist system certified by academic societies. Since a model project for a certified specialized qualification system for care workers was completed last year, and the system is expected to be established in the near future, these qualifications are thought to be the "driving force for dementia care". Training and qualifications for dementia care must lead to the provision of collaborative, efficient, and effective care in care settings. Issues that require improvement include systems for evaluating and applying the knowledge and skills obtained from specialized training and qualification systems in care settings, as well as establishment of on-the-job training (OJT) systems.
