Docetaxelを含む多剤併用化学療法後に11q23異常を伴う治療関連白血病を発症した進行前立腺癌の1例  [in Japanese] THERAPY RELATED LEUKEMIA WITH 11q23 ABNORMALITY INDUCED BY CHEMOTHERAPY CONSISTED OF DOCETAXEL FOR ADVANCED PROSTATIC CARCINOMA: CASE REPORT  [in Japanese]

Abstract

11q23に位置するMLL遺伝子を巻き込む均衡転座は治療関連白血病に認められる.原因となる薬剤はtopoisomerase II阻害剤がほとんどとされているが,docetaxelによる治療関連白血病の報告は極めてまれである.我々はdocetaxelにより治療関連性白血病を発症したと考えられた進行前立腺癌を報告する.症例は69歳男性.ホルモン不応性前立腺癌の局所再発および肝転移に対し,docetaxel,リン酸エストラムスチンおよびcarboplatinによる3剤併用化学療法を行った.前立腺癌の進行は認めなかったが,docetaxel開始からわずか10ヵ月後に急性骨髄単球性白血病が出現した.染色体検査で9番染色体短腕と11番染色体長腕の転座を認め,化学療法による治療関連性白血病と診断した.原因薬剤として臨床経過と転座所見の文献的な背景を考えるとdocetaxelの可能性が最も高いと考えられた.

A balanced translocation involving 11q23 (MLL gene) could be observed in therapy related leukemia (TRL) patients generally treated with topoisomerase II inhibitors. Few reports have been published on TRL following docetaxel administration. Herein, we report a patient who developed acute myelomonocytic leukemia (AMMoL) with t (9; 11) (p22; q23) following chemotherapy mainly consisted of docetaxel for advanced prostatic carcinoma. A 69-year-old man was treated with a systemic chemotherapy containing docetaxel (total dose=585mg) for hormone-refractory metastatic poorly differentiated prostate carcinoma. Although, no disease progression of the prostatic carcinoma was observed, AMMoL with t (9; 11) (p22; q23) developed only ten months later from the administration of docetaxel. Docetaxel was considered to be the cause of TRL because of the presence oft (9; 11) (p22; q23) translocation and clinical course.
