Veno-occlusive disease associated with a periportal edema in a child diagnosed with Wilms' tumor

Abstract

We experienced veno-occlusive disease of the liver (VOD) that developed ascites without jaundice. The patient was treated for a left-sided Wilms' tumor without metastasis according to the Japan Wilms Tumor Study Group (JWiTS) protocol after operation. High fever, severe abdominal distention and 16% weight gain developed on the fourth day after the second cycle of actinomycin-D. Contrast-enhanced computed tomography (CT) revealed massive ascites associated with periportal edema. The patient was diagnosed as VOD. Seven days after treatment with steroids, ascites disappeared and deterioration of liver function without jaundice developed thereafter. A follow-up CT confirmed the disappearance of the periportal edema. The patient underwent chemotherapy without actinomycin-D and survived without any recurrence of disease.
