1998年に障害者のケアマネジメント事業が沖縄で開始されてから8年目になる。障害者ケアマネジメントの実践はこれまでの沖縄の障害者福祉を大きく変えるものである。障害児(者)地域療育等支援事業を中心にその実践を紹介する。名護療育園では療育園の機能とスタッフを活用して地域の障害者に対する支援が行われてきた。また名護市を中心とする北部圏域では身体・知的・精神3障害の支援センターとさらに就業・生活支援センターが沖縄で最も早く設置され、障害の種別をこえたネットワークが形成された。糸満市のみなみの里では沖縄で初めて知的障害者のケアマネジメント施行事業が実施され、ケアマネジメントの実践が積み重ねられた。本島中部にある中城村のグリーンホームでは地域の中に入りニーズを掘り起こす活動が行われ、今では養護学校からの相談を含め多くの相談が入るようになり、一人では対応しきれない状況が生じている。石垣市の八重山育成園でも、竹富、与那国など離島も含む訪問活動を積み重ねるとともに、同時に身体障害者の支援センターも設置し身体・知的の障害者に対して統合的に相談活動を進めてきたが、さらに現在は精神障害者生活支援センターも統合する方向を目指している。ケアマネジメント活動のなかで、重症心身障害児通園事業(糸満市)、児童デイサービス(うるま市)、障害者福祉サービスを有償で行うNPO法人設置(石垣市)等新しい社会資源も作り出されている。本稿の最後に、要求の掘り起こしという初期の段階から、増大するニーズに対応する安定した組織の確立や地域の相談ネットワークの確立が求められていることなど新しい段階に入ったケアマネジメントの今後の課題について提起した。
The social activities of care management for disabled persons in Okinawa prefecture began in 1998. Care management for disabled persons has brought important effects on the advancement of normalization in Okinawa. In this report, I introduce care management activities for mentally disabled adults and children in Okinawa. In Nago Ryouikuen (facility for severe disabilities), staff and facility equipment have been used for disabled persons in the community. The northern area of Okinawa Island including Nago City founded the earliest networks of disabled persons (persons with physical disabilities and mental disorders including mental retardation) in Okinawa. Minami-no-Sato (facility for mentally disabled adults), in Itoman City began the first trial programs of care management for mentally disabled persons and disabled children in Okinawa. The care management practices in Minami-no-Sato have served as models for other areas. The care manager in Green-Home (facility for mentally disabled adults), in Nakagusuku Town has been visiting people in their communities during the day. Many new needs of disabled persons have been found. The welfare needs of children of school-age (including high schools for the mentally disabled) are also increasing and the care manager is obliged to cover over 100 cases. The care manager of Yaeyama Ikuseien (facility for mentally disabled adult persons) in Ishigaki City has been visiting people including Taketomi Island, Iriomote, Yonaguni and other small islands. Yaeyama Ikuseien also established a support center for physically disabled persons (Muyurukan). Muyurukan and Maaru (support center for mentally disordered persons) have been run together and now are aiming to merge. Finally, I make several suggestions for the improvement of the care management of disabled persons in Okinawa.
