In order to assess the association between endogenous psychosis and fever, we conducted a case history study in regard to fever during the hospital stay of 1,466 inpatients in our department. There were 237 patients who fulfilled our original fever criteria. Eighty-nine of these patients had fever of unknown origin (FUO) and a diagnosis of endogenous psychosis. diagnostic category, there were 29 patients with depression, 21 with mania, and 22 with schizoaffective disorder, manic-type. When these patients were compared to 1,009 patients with a diagnosis of endogenous psychosis without fever, the proportion of patients with schizoaffective disorder, manic-type, was found to be statistically significantly higher. Thirty of the FUO patients fulfilled Levenson's diagnostic criteria for malignant syndrome, and 6 of these patients fulfilled Adityanjee's criteria. Multifactorial assessments were also conducted for psychosomatic manifestations in these 89 subjects. There were mutual interrelationships between the height of the fevers, the extrapyramidal tract manifestations, autonomic nervous system manifestations, degree of consciousness transformation, and the catatonic symptoms. These manifestations, which almost perfectly coincide with Adityanjee's four criteria, were found to possess close symptomatic associations which characterize severe FUO in patients with endogenous psychosis.
