抄録

全国の統合失調症精神科入院患者の拒薬概要,及び看護師の拒薬対応の実態を明らかし,服薬行動を促進するための有効な対応方法について検討することを目的とした。方法は2段階とした。聞き取り調査で内容などを決定し,それを基にアンケート調査を全国400ヶ所の病院に郵送法で実施した。その結果,以下の3点が明らかになった。(1)薬の必要性を説明するだけでなく,具体的目標を示すことが有効である。また患者の気持ちを考慮したQOLから長期的な視点で説明する。(2)患者と看護師との信頼関係構築が重要であり,担当する看護師以外へ拒薬対応を依頼することも有効である。(3)患者の状況を観察し,それに沿って不安を軽減する効果のあることをはっきりと具体的に説明することで内服を促すことが有効である。(2)と(3)の結果については先行研究結果と同一方向であった。(1)については看護の視点から新たに確認されたことと考える。なお,多様な事例を一体で扱ったことが本研究の限界であり,今後の適切な分類の下での調査が求められる。

This study is based on a survey conducted to schizophrenic patients hospitalized in psychiatric wards who refuse to take medication, along with nurses' attitudes toward their refusal, in order to research effective ways for nurses to encourage those patients to willingly follow the medication.We performed the study on two stages. In the first stage, we decided on the questionnaire items of inquiry survey through hearing investigations. In the second stage, we performed the inquiry survey on 400 hospitals by mail across Japan.The analysis of the results is as follows: (1) it is effective not only to explain the necessity of the medication in general but also to explain how it relates to each patient's own goal, and also in conjunction with his/her long-term goal, where QOL must be preserved, (2) it is sometimes effective to change from one nurse in charge to another, if this can better establish the patient's confidence in the relationship, and finally (3) while observing the patient's attitude in detail, it is effective to give full explanation concerning effects of the medicine in order to reduce his/her worry and encourage him/her to accept medication.As for (2) and (3), the analysis supports the result of preceding research. However, (1) can be identified as a new phase being verified from the nursing point of view. The future challenge will be the improvement of data analysis:all the cases should be regrouped for more appropriate classification in order to gain benefits from the research.
