• A Case with Dental Implant Treatment in Combination with Tooth Extrusion Performed before Two Anterior Teeth Replacement for Preservation of Inter-proximal Papillae in the Maxilla

抄録

A 54-year-old man complained of increasing mobility of his maxillary lateral and central incisors. Clinical examination revealed a vertical fracture on the buccal root portion of each tooth. Vertical probing depth was 8 mm around the central incisor, and 9 mm around the lateral incisor. A successful esthetic and functional restoration was achieved by a combined use of immediate implant placement, and forced eruption of a fractured tooth. Methods : The lateral incisor was extracted and immediate implant placement with GBR was performed. The implant was successfully osseointegrated and temporary restoration was delivered. Subsequently, a metal hook was attached to the central incisor, four brackets were attached to the upper anterior teeth and the implant crown, and orthodontic extrusion was started using an elastic band. After one month, planned extrusion was achieved, and the central incisor was extracted. Three months later, an implant was placed into the central incisor site. The final prosthesis was completed 6 months later, followed by clinical maintenance. Results : The papillae were augmented by orthodontic extrusion. The peri-implant soft and hard tissues were manipulated and maintained in a stable state. Conclusion : We successfully reconstructed the interdental papillae and replaced the deficient teeth with immediate and conventional implants. During 6 years of follow-up, the implant and peri-implant structures have remained stable and healthy.
