Abstract

In Japan, transition support from child to adulthood provided trough "Kobetsu shien keikaku(Individual transition plan)" at school. This brief note points that there is a tendency to focus on working and is a tendency for teachers and parents to make the plan. Under the Revision law, the workers of social service can support disabled child and adult when they use social service, so the workers have to help them to consider various choices about their future, and make the plans reflecting their determination.
