Public health nurses belonging to municipalities (hereafter, municipal public health nurses) play a central role in promoting activities on the forefront during disasters. A case study of nine municipalities affected by the Great East Japan Earthquake was conducted, concerning the aspects of reconstruction of community healthcare activity systems, specifically in relation to health centers (n=7) or prefectural offices (n=2), during the approximately one-year period after the disaster.In this report, the characteristics and issues involved in the public health nursing activities of municipal public health nurses during the disaster were described.Relationships of trust between public health nurses and community residents, as well as local parties, and familiarity with the area and knowledge of community resources, local culture, and customs provided the foundations for activity promotion in the public health nursing activities of municipal public health nurses during disasters. On the other hand, several issues, including the rapid transition from medical activities to public health activities and reconstruction of organizational systems that enable implementation of community-based activities, were also identified. Important measures for addressing these issues included the following: integration and unified management of public health nurses, who are normally dispersed more than one department; the presence of public health nursing manager who play a key role in activity promotion; the roles of vice manager; activity promotion through organization, and repeated re-organization of activities according to changing needs; and the roles of external support providers backing the public health activities of municipal public health nurses.In this report, psychological care for municipal public health nurses, who, despite being affected by the disaster themselves, are in a position to provide continuous support to affected residents was also discussed.
