Cognitive Behavioral Therapy for Chronic Dizziness

Abstract

Background : Chronic dizziness is assumed to be the clinical condition based on a vicious circle of psychophysiological process between balance system and anxiety. Cognitive behavioral therapy is one of the most promising treatments for chronic dizziness, however, there is no standard treatment program that shows long-term effectiveness. The purpose of this study is to develop a cognitive behavioral treatment program for chronic dizziness and to examine its long-term effectiveness preliminarily. Subjects: 20-to-70-year-old patients at a special dizziness clinic, with a chief complaint of dizziness that lasted more than three months and that was not explained adequately by organic factors. Method : Patients received 4 or 5 weekly sessions of cognitive behavioral group therapy. Handicap due to dizziness (Dizziness Handicap Inventory: DHI) was measured for six months after the treatment. Results Twenty-two patients were enrolled. There was a significant improvement between pre- and post-treatment, and the improvement continued steadily up to six months. The effect size from pre-treatment to six months later was 1.09, and similar to that in the previous study. The remission (defined here by 14 or less DHI score) rate at six months after treatment was 45.5%. Conclusion : The results suggest that this treatment program has long-term effectiveness. To determine it, further clinical trial should be required.
