平成19年度から本格的に特別支援教育が始まっている。「障害者基本計画」「重点施策実施5か年計画」(新障害者プラン)では障害のある人を生涯にわたって支援する「個別の支援計画」の策定が提唱されている。就学前から小学校への移行に伴う就学相談・指導は各市町村単位の取り組みはあるものの、平成14年度からの新しい就学基準に沿った就学指導ができていると言い難い。そこで望ましいと思われる就学支援シートを作成したいと考えている。その前段階として今回は、就学支援の実践を通して成果を述べている東京都のモデル2市、福井県の養護学校1校、島根県松江市の取り組みを教育と医療の連携という視点から比較、検討したので、報告する。

Special support education started in full scale in 2007. The decision to implement an "Individualized Support Program" supporting people, who deal with lifelong handicaps, is advocated by the "Handicapped Person Basic Plan" and the "Emphasis Measure Execution Five Year Plan" (also known as the new "Hndicapped Person Plan"). However, though we have counseling and guidance for entering school, schools providing special support education (especially for the transition from preschool to elementary school) for each city, town and village, it is possible to say this is not enough. So, we would like to propose a "Supporting Sheet" for entering elementary school. As the former stage we would report the results retrieved by comparing and discussing four examples: programs in two cities in Tokyo, a school for the handicapped in Fukui, and a program in Matsue City in Shimane. These four examples will be discussed and compared through their activities related to support aspects highlighting the cooperation between education and medical care.
