[症例報告]Malignant Granular Cell Tumor of the Chest Wall : A Case Report and Brief Review of Recent Literature

A 59-year-old man with a 5-year history of hemodialysis for renal failure was incidentally detected to have a mass in his chest wall when the patient presented at our hospital for an examination of erythropoietin-intractable anemia in June 1999. The mass was located on the 10th rib within the chest wall. Ultrasonography (US) revealed a hypoechoic mass measuring 5 cm in diameter. Computed tomography (CT) and magnetic resonance image (MRI) showed a circumscribed mass identical to the skeletal muscle based on the imaging findings. The mass together with the adjacent skeletal muscle was radically removed. On bisection, the tumor measured 5×3 cm in size, and was encapsulated, firm, solid and tan-yellow. The tumor was composed of solid sheets or fascicles with fairly pleomorphic, polygonal and focally spindle cells with an abundant eosinophilic, granular cytoplasm and vesicular nuclei which contained large nucleoli. The tumor cells spread to the muscle, to the adjacent adipose tissue and the vessels. No area of necrosis was observed. Seven mitotic figures 50 HPF were present. Based on thses findings, the tumor was diagnosed to be malignant granular cell tumor (malignant GCT). Cytoplasmic granules in the tumor cells were positive according to the findings of PAS diastase staining. Immunohistochemically, the tumor cells were positive for S-100 protein, NSE, and vimentin. The patient is at present doing well about 2.5 years after surgery without any evidence of recurrence or metastasis.
