性暴力被害は、犯罪被害のなかでも最も外傷後ストレス障害(PTSD)を起こしやすいことが知られており、自責感や周囲からの二次被害によって回復が妨げられることが多い。PTSD への治療として実証されているものの一つである曝露療法(Prolonged Exposure;PE 療法)は、PTSD の心理教育、実生活内曝露、想像曝露、宿題等からなる構造化されたプログラムであり、外傷体験に関連した回避への曝露と馴化により症状を軽減させる。本稿は、性暴力被害によりPTSD を発症していた成人女性に対してPE 療法を施行した結果をまとめたものである。評価には、CAPS、IES-R、PDS、SDS、BDI-II、DES を用いた。15 回のセッションによってPTSD 症状は消失し、関連症状である解離症状が軽減した。

The rape against women is known to be caused to the PTSD most in any crime victims, and is disturbed the recovery due to damage from the feeling of self-blame and often taken second-victimizations by another people. The Prolonged Exposure (PE) is one of the evidence-based treatment for PTSD, is a structured program that consists of the psychoeducation, the exposure in vivo, imaginary exposure and homework. The symptom is reduced by the exposure and the adaptation in memory of fear related to the traumatic experience. This is the case study on the treatment for adult female client who was rape by PE. CAPS, IES-R, PDS, SDS, BDI-II and DES were used to evaluate the symptoms of PTSD, comorbid depression and dissociation prior to and following treatment. The symptoms of PTSD and dissociation disappeared by 15 sessions. Chronic PTSD was markedly improved. The PE therapy was effective in the reduction of PTSD.
