Wilms’ tumor is the most common malignant renal tumor in children. According to years of research efforts by international multi-institutional cooperative groups, many of its characters have been well analyzed and are now largely clarified. Wilms’ tumor in adults, however, is very rare, and has a worse prognosis than in children. Due to its rarity, there have been only a few comprehensive reports about adult type tumors. Because there are currently no recommended treatment strategies for these adult patients, we commonly treat them with protocols developed for children. We herein report a case of adult Wilms’ tumor in an 18-year-old male. A right renal tumor was excised by urologists at the referring hospital. The histopathological examination revealed that the tumor was a mixed type Wilms’ tumor. Therefore, he was referred to our pediatric division to undergo postoperative treatments. Although we treated him with a protocol for children, the tumor relapsed in the lungs with multiple metastases. After that, he was treated again with whole-lung irradiation and more intensive chemotherapies. During the treatment period, he suffered from both nausea and neurotoxicity due to the chemotherapy. His disease is now in remission. Since the observation period is still short, the curative effect of the treatment is unknown.
