本研究は言語的コミュニケーションに難しさのある高齢者のこころの内面を知ろうとする看護師のかかわりを明らかにすることを目的とした。看護管理者からスピリチュアリティを大切にした看護を実践しているとの推薦があり、本人の同意が得られた20 人の看護職者に対し、「言語的コミュニケーションがとれない高齢患者さんあるいは訴えがない患者さんの“ こころの内面” はどのようにして把握されていますか」の質問を行った。この結果、【その人なりの意思表出のサイン、反応をキャッチする】【変化を感じ取る】【意思表出を引き出す意図的なかかわり】【環境づくり】【きっかけになる重要他者】【きっかけとなる情報を探す】【その人の意思表出の力を信じる】【その人のこころの内面を探し求め試しながら、確認する】【他のスタッフとの協働・共有】【看護師の感受性】の10 つのカテゴリーが抽出された。

The purpose of this qualitative study was to clarify nurse involvement in / dealings with the inner world of elderly people with great difficulty in verbal communication. Interviews were conducted with 20 nurses who were recommended by the directors of the institutions they belonged to because of their respect for spirituality and agreed to participate in the study. All the participants were asked： “How do you try to grasp the inner world of elderly patients who have great difficulty communicating verbally?” As a result, the following 10 categories emerged： “detecting each patient’s own signs of self-expression,” “perceiving changes,” “eliciting self-expression from patients intentionally,” “creating an environment,” “making approaches to significant others,” “seeking information to engage patients in conversation,” “knowing that patients are able to express themselves,” “sounding out and identifying patient thoughts and wishes,” “collaborating and sharing with other staff,” and “having a receptive mind.”
