本研究は、本学の助産課程履修者の大学卒業時における分娩直接介助実践能力の到達度を明らかにすることを目的とした。本学助産過程履修者で、卒業生の分娩直接介助実習評価表40 事例を対象とし、厚生労働省（2012）により出された「助産師に求められる実践能力と卒業時の到達目標と到達度」の、大項目である分娩期の診断とケアを　参考に、本学の分娩直接介助実習評価表の項目を対応させ、到達度を明らかにした。全項目の平均得点は、7 例目から平均得点が上昇しており、卒業時に2.45 点（“ 一人でできた” と“ 指導を受け一人できた” の間）となり、指導を受けながら、段階を経て分娩直接介助実践能力を高めていた。項目別での到達度で、10 症例を通してすべて“ 指導をうけ一人でできた：２点” に満たない項目は、分娩進行状態の診断、分娩進行に伴う産婦と家族のケア、児頭娩出介助、躯幹娩出介助、胎盤娩出、の５項目であった。今後の課題として、到達目標の見直し、7 例目までに教員との面談の場面で、これまでの分娩介助を振り返り課題を明確にすること、シミュレータを活用し、シナリオをいくつか想定して行うことなどの学びの更なる工夫が、到達度を高めることにつながると考えられた。

Objective：To investigate the level of attainment in competence of direct birth assistance among students in a four-year university midwifery program at the time of graduation. Method: Forty cases of practice evaluation sheets of direct birth assistance for midwifery students were investigated. The attainment levels were demonstrated by adjusting the items of the practice evaluation sheets with reference to “diagnosis and care of patients in intrapartum period,” which was a major item of “practical competence required for midwives and attainment target levels at the time of graduation” published by the Ministry of Health, Labour and Welfare in 2012.Results：An average score of all items increased from the seventh labor assistance case and reached 2.45 points at the time of graduation. The scores indicated that the students improved their performances in stages with guidance. In the achievement levels by items, five items did not reach 2 points throughout the ten-time experiences of birth assistance. These items included the diagnosis of the stages of birth progress, care for parturients and their families according to the birth progress, and assistance in expulsion of the fetal head, trunk, and placenta. The findings suggest that review of attainment target is required.
