Sixteen cases of anal and perianal carcinomas have been experienced in our clinic for the past 11 years, occupying 8.6% of all the rectal and anal carcinomas. In spite of the rarity, anal and perianal carcinomas show variety in their pathological manifestation. This is due to the specificity and complex composition of such integuments of the area as ano-derm and cloacogenic zone.
1. Adenocarcinoma tubulare (1 case) It is very rare compared with that rectal cancer.
2. Adenocarcinoma epidermoides (2 cases) The histology shows adenocarcinoma with the characteristics of epidermoid carcinoma.
3. Colloid carcinoma (6 cases) It is characteristic in its occupation in perianal or perirectal regions and in the symptoms of tumor-formation and in the symptoms of tumor-formation and gelatinous secretion. Accordingly, most of them had been erroneusy diagnosed and treated as fistulas. The histology shows cysts filled with mucin and communication with anal gland in 2 cases. Prognoses are favorable.
4. Epidermoid carcinoma (4 cases) Histologically, various degrees in maturity are ob-served among the cases. However, that does not seem to influence much of their prognoses. In their peripheral areas, infiltration proceed confined in the epidermis.
5. Basosquamous cell carcinoma (2 cases) Infiltration of nonkeratinizing basaloid nest is observed.
6. Basal cell carcinoma (1 case) Formation of " rodent ulcer " in perianal area and no tendency to metastasize are the characteristics. In our case, it is pigmented and was errored as malignant melanoma.
All the 16 cases, except the last regrettably, are progressed cases. Some causes of the retardation in finding are due to patient's erroneous self-diagnosis and treatment of piles and the others are due to medical misdiagnosis and repeated operation of fistula in vain.
