Although rectal carcinoid tumors are uncommon lesion, they have been reported with increasing frequency. For the past two years, five cases of rectal carcinoids were experienced, and of those, four cases were found among 1614 patients who were performed routine proctosigmoidoscopy at Isehara Kyodo Hospital.<BR>Between 1967 and 1982, 436 patients with rectal carcinoids were reported in Japanese literature. The study was undertaken to determine the relationship between the tumor size, the depth of invasion and metastatic spread. In this series, the age varied from 13 to 81 years, with an average of 47.7 years. The ratio of men to women was 1.49 : 1. Eighty-five percent of the lesions were located on 4 to 9cm from anal verge. About four fifth of the tumors were less than 2cm in diameter. Twenty-seven of the 35 patients (77%) with the tumors larger than 2cm had evidence of metastasis and 22 of the 26 patients (85%) of those tumors invaded to muscular layer. Whereas, only 15 of the 190 patients (7.8%) with the tumors less than 2cm had metastases and 20 of the 156 (13%) had muscular involvements.<BR>But, among the tumors less than 2cm, the lesions measureing 1.1 to 2.0cm had relatively high incidence of metastasis (23%) compared with the lesions less than 1.0cm (2.8%). Therefore, we recommended those tumors must be resected locally first to evaluate the histological findings and to select appropriate indication of radical resection.
