Since the Silverstein's first report(1966) on elevated protein concentration in the perilymph of the patients with acoustic tumor, it appears that "diagnostic labyrynthotomy" can be useful and worthwhile in certain cases of unilateral sensorineural hearing loss where acoustic neurinoma or Meniere's disease is suspected.<br>The objective of the present study is to present the data on chemical analysis of the inner ear fluid obtained from experimental animals in which arachnoiditis of the internal auditory canal cistern was produced and also to discuss the pathogenesis of elevated perilymph protein concentration in connection with histopathological findings.<br>The results of the animal experiment were as follows:<br>1) The arachnoid of the internal auditory canal cistern was markedly thickened and adherent by proliferated fibrous tissues, and subsequent subarachnoid space obliteration was observed.<br>2) Elevation in protein concentration in the perilymph of the inner ear was observed particularly in the side of severely affected arachnoiditis, while the concentration of Na and K were within normal range.<br>3) In almost half of the animals with arachnoiditis, hearing loss of retrocochlear type was observed on computer audiometry.<br>The result of the present study suggested that elevated protein concentration in the perilymph might represent a considerable obstruction of the subarachnoid space and a subsequent severe C.S.F.circulatory disturbance in the internal auditory canal cistern.
