In order to study the glucose tolerance in acute hepatitis, blood sugar and serum immunoreactive insulin (IRI) were determined during 100g-oral glucose tolerance test (GTT) in 51 patients with acute hepatitis, who have neither family history nor past history of diabetes mellitus and all have normal fasting blood sugar levels.<BR>Hepatic biopsy was performed in all patients.<BR>In acute hepatitis, glucose intolerance was frequently encounted and about 85% of patients showed glucose intolerance in acute stage of acute hepatitis.<BR>The mean level of 30'ΔIRI/ΔBS in acute stage was significantly lower than that of normal subjects, but, this parameter returned to normal after recovery stage of acute hepatitis. However, in the group in which the level of 30'ΔIRI/ΔBS was low (<0.4) in acute stage, this parameter increased only slightly after recovery.<BR>Then, there was significantly negative correlation between the level of 30'ΔIRI/ΔBS and liver cell necrosis.<BR>In addition, the low level of 30'ΔIRI was observed in patients with severe liver cell necrosis.<BR>This study suggests that glucose intolerance in acute hepatitis is due to not only hepatic damage but also pancreatic β-cell impairment.<BR>In clinical course of acute hepatitis, the level of 30'ΔIRI/ΔBS of healed cases was significantly higher in acute stage than that of cases of persistent or chronic hepatitis. In addition, the value of 30'ΔIRI/ΔBS showed remarkable increase in the recovery stage. From these results, it was conchlded that 30'ΔIRI/ΔBS was one of the useful index with regard to prognosis of acute hepatitis.
