The authors histologically examined 143 liver biopsies taken at institutions participating in an integrated study on alcoholic liver disease (Chairman, Prof. Akira TAXADA) supported by the Ministry of Education, Science and Culture. We divided these 143 cases into three groups, a virus marker positive-alcoholics group containing 35 cases, a virus marker negative-alcoholics group including 81 cases and a non-alcoholics group of 27 cases, and compared them with each other. Characteristic features in alcoholics were stellate fibrosis, pericellular fibrosis, dense fibrosis and ballooning (p<0.001). We found moderate to severe inflammation confused with chronic viral hepatitis in 16 cases of the virus marker negative-alcoholics group (19.8%) and concluded that further study was needed to define its cause and to clarify the possibility of alcohol induced chronic hepatitis. Discrepancies were found between our histological diagnoses and clinical diagnoses made at each of the institution on alcoholic hepatitis.
