Seventy nine small liver nodules, less than 2cm in diameter, rescted for the clinical diagnosis of HCC are reported in detail.<BR>About half of the nodules showed obscure margin tumor macroscopically. The other half were distinct margin nodules. At the margin of the obscure nodule, cancer cells showed replacing growth, and histological atypia of cancer cells was so mild that it was very difficult to differentiate cancer cells from surrounding hepatocytes. However, these cancer cells invaded portal tracts frequently. On the other hand, the distinct margin nodules showed a distinct cellular atypia like that seen in the large advanced HCC. Around the center of the obscure nodular tumors, atypical cells appeared forming small nodules or scattered randomly. As the central atypical cells proliferated, pre-existing well differentiated cancer cells that surrounded them were compressed outward to the edge of the nodule. The other type of obscure nodule showed expanding growth as a whole, and compressed surrounding hepatocytes with capsule formation resulting in a distinct margin like the advanced HCC.<BR>These findings suggests that the obscure margin nodules represent early stage of HCC, and the distinct margin nodules represent advanced HCC, even if they are small.
