Recently, there are reports that the echogenecity of a minute hepatocellular carcinoma will change from hyperechoic to hypoechoic while growing and that fatty metamorphosis of the hepatocytes would be characteristic change of a minute hepatocellular carcinoma. We report here such a case that supports these possibilities.<BR>A fifty-nine-year-old Japanese male was admitted to Kyushu University Hospital because of a small mass in the right lobe of the liver which was detected by ultrasonography (US). The mass appeared as a hyperechoic nodule of 8mm in diameter. Fine needle biopsy disclosed the specimen consisted of relatively large cells with fatty metamorphosis. The diameter of the mass increased to 12mm and the echogenecity turned hypoechoic one and a half year later. Then, the mass grew to 19mm four months later. Hepatic arteriography failed to demonstrate the mass. However, as computed tomography taken after the lipiodol infusion disclosed a lipiodol deposition, resection of the mass was performed. Histology revealed well-differentiated hepatocellular carcinoma (Edmondson I) with fatty metamorphosis in the periphery of the tumor.
