抄録

A case of drug-induced hepatitis with multinuclear giant cells in adult was reported. A 62-year-old woman was admitted to the hospital because of nausea and jaundice after administration of cefradine and ibuprofen. On the 14th hospital day, peritoneoscopic examination and liver biopsy were performed. The histological findings showed widespread degeneration and mild necrosis of liver cells. Moreover many multinuclear giant cells concluding bile pigments were seen in the centrilobular areas. Serological tests for HBvirus, HAvirus and EBvirus were negative, whereas lymphocyte stimulation tests for cefradine and ibuprofen were positive. And as positive antinuclear antibody and elevated IgE titer were sometimes seen in drug-induced hepatitis, we made a diagnosis of drug-induced hepatitis due to cefradine and ibuprofen. Liver function tests were improved and antinuclear antibody became negative by prednisolone therapy. We couldn't enough revealed why multinuclear giant cells were seen in this drug-induced hepatitis and appearance of these cells preceded or followed by cholestasis. But this case is thought to be a very interesting case to investigate in multinuclear giant hepatic cells.
