A case of malignant fibrous histiocytoma (MFH) of the pleura is presented. A 62 year old woman was admitted because of cough and general malaise. Chest X-ray film showed left pleural effusion and thickening. Though she received antituberculous agents, she died about 3 months after admission. Autopsy revealed the left pleural thickening which was diagnosed as MFH. The tumor had metastasized to many organs; lung, liver, stomach, duodenum, kidneys, adrenal glands, thyroid, heart, lymphnodes. Histologically, the tumor consisted mainly of fibroblast-like cells, histiocyte-like cells and bizarre giant cells, which were arranged in a storiform pattern. The mesothelioma was neglected because PAS and Alcian Blue staining were negative and immunohisto-chemical technique for α1-antichymotrypsin was positive.
