We report a case of verrucous carcinoma of the lower lip successfully treated with superselective intra-arterial infusion of peplomycin. A 29-year-old man noticed a tumor on his lower lip about two years before his first visit. He had atopic dermatitis and had been smoking cigarettes from the age of 20. He was diagnosed with verruca vulgaris and treated with liquid nitrogen at another hospital. Despite the treatment, the tumor enlarged; for this reason, he was referred to our hospital. Physical examination revealed a cauliflower-like tumor 20×16mm in size on his lower lip. Histologically, it was diagnosed as verrucous carcinoma. Since the tumor involed cosmetically critical area, a wide excision would have led to unacceptable disfigurement. Thus, the treatment with superselective intra-arterial infusion of peplomycin was selected to maintain his quality of life. Eighteen months after the therapy no recurrence had been noticed.
