The aim of this study was to assess the relationship between the incidence of rotator cuff tear and the coverage of the humeral head with coracoacromial arch in the sagittal plane. Forty-two shoulders from 21 embalmed cadaveric specimens were examined macroscopically. Their mean age was 83 years old. We divided these shoulders into two groups; i.e., tear group: shoulders with full-or partial-thickness cuff tears, normal group: shoulders without cuff tears. Eight shoulders (19%) were included in the tear group, whereas the rest, 34 shoulders, were included in the normal group. After removal of the Deltoid muscle, the specimen was attached to the special device. Photographs taken from the lateral side were imported to the computer. A circle was drawn along the coracoacromial arch, and the horizontal line was set as a criterion line. Then, the angle of the acromial coverage as well as the coracoacromial arch was compared between the two groups. The width of the coracoacromial ligament in the tear group (36 degrees) was relatively smaller than that in the normal group (48 degrees, p=0.08). The acromial coverage in the tear group was significantly greater than that in the normal group (96 degrees and 88 degrees; p<0.01). No significant differences were seen in the total coverage angle of the coracoacromial arch in the two groups. These results suggested that the small coracoacromial ligament as well as too much acromial coverage seemed to correlate with the incidence of rotator cuff tear.
