A method for endoscopic removal of foreign bodies in the bronchial tree using the Fogarty balloon catheter is described. The patient was a 13-month-old boy whose brother had stuffed small stones into his mouth and then thrust them down. Dyspnoca and cyanosis developed at the time of the accident. On examination, there was no respiratory distress; however, X-ray examination revealed that there were two stones in the left bronchial tree and ten or more stones in the stomach.<br>The larger stone in the left main bronchus was removed by a grasping forceps through a ventilation bronchoscope. The smaller stone fixed in the lower lobe bronchus could not be removed endoscopically. Further endoscopic removal was successfully performed using a Fogarty balloon catheter, which is 0.8mm in diameter with a balloon at the top. Under fluoroscopic guidance, the catheter was passed through a gap between the stone and the bronchial wall to the periphery and the balloon was filled with contrast material. By withdrawing the catheter the stone was pulled up to the main bronchus and was removed successfully by a grasping forceps.<br>Because of this experience, we recommend this procedure before considering thoracotomy for the removal of a foreign body which cannot be removed by the usual endoscopic procedure.
