抄録

  本邦に導入されている第2世代および第3世代抗CCP抗体試薬9種類について，関節リウマチ（RA）89例，RA以外の膠原病（非RA）121例，変形性関節症（OA）47例，慢性炎症性疾患142例および健常人168例を対象とし，精度および抗体価の相関や判定一致率を比較検討した．9種類試薬の同時再現性は変動係数（CV）0.7～8.5%，日差再現性はCV 0.6～8.3%であった．第3世代試薬のカットオフ値を50 Unitsに変更すれば，9種類全体で96.8～99.6%と高い一致率になり，RAの感度は75.3～78.7%，非RA，OA，慢性炎症性疾患および健常人での特異度はそれぞれ93.4～97.5%，97.9%，96.5～98.6%，98.8～100%と優れた診断精度が確認できた．抗体価の相関性では第2世代試薬の一部と第3世代試薬を除けばrs=0.95～0.99と比較的保たれていたが，第3世代試薬は異なる抗体価の動態を示していた．本邦に導入されている9種類の抗CCP抗体検査試薬はいずれも日常試薬として使用できる性能を有していたが，2010 RA分類基準の高力価（基準値上限の3倍以上）検体については試薬によって不一致例が存在し，スコアリングに差がでる可能性があり問題である．<br>
