抄録

胆道疾患に対するERCPは，低侵襲で安全な手技として広く普及している．選択的胆管挿管は必要不可欠であるが，時に困難例も存在する．こうした胆管挿管困難例に対して，近年，EUSガイド下ランデブー法（EUS-RV）の有用性が報告されている．EUS-RVはEUSガイド下に上部消化管から胆管を穿刺し，ガイドワイヤー（GW）を順行性に穿刺針を通して，胆管・乳頭部を介し十二指腸内に留置した後，十二指腸鏡に入れ替え，留置されたGWを利用し選択的胆管挿管を行う手技である．文献報告では，EUS-RVは安全で効果的な代替法とされるが，失敗に終わった際には合併症を最小限とするため，経皮的・外科的処置の準備も必要である．<br>
