From 1975 to 1995, 352 cases were implanted or reimplanted permanent pacemaker in our clinic. Eight out of 352 cases were treated for pacemaker system infection during the same period. Three cases were treated with total removal of the pacemarker system and the other five cases were treated with partial removal. The treatment was successful in all cases. In pacemaker infections, all foreign materials must be removed to eradicate associated infection. But total system removal is difficult when the lead is firmly fixed for a long term. In our guideline, we pursue the treatment of infections of the pacemaker step by step. We treat those patients conservatively after a partial removal of the system as the first approach, and therafter removal of the total system is performed under cardiopulmonary bypass, if their infections are uncontrollable. We have attained good results so far.
