To evaluate the present status of perioperative blood transfusion in our hospital, we analyzed in detail the types of blood transfusion during pre-, intra- and post- operative periods. During one year, perioperative blood transfusions (excluding autologous ones) were performed in 328 out of 2, 232 surgical patients. Of these 328 patients 56% (183 cases) were transfused in both the intra- and post-operative periods. 28% (81 cases) and 14% (45 cases) was transfused in the post- and intra-operative periods, respective-ly. Pre-operative blood transfusions (6%; 19 cases) were done in patients with high risk or hemorrhagic shock. The amount of pre-, intra- and post-operative blood transfusions was 92, 1, 130 and 734 units, respectively. The critical concentration of hemoglobin at which blood transfusion was introduced was 9.5g/dl, and the mean volume of blood transfusion in most cases was equal to that of total blood lost during the intra- and post-operative periods. From these results, we recognized that perioper-ative blood transfusion should be done in accordance with the generally accepted guideline for proper blood tansfusion, and that it is necessary to reduce post-operative blood transfusions as well as intra-operative ones in order to reduce the total amount of perioperative blood transfusion.
