This paper describes a case of occult carcinoma of the breast which was diagnosed by right axillary lymph node metastasis, together with a review of the literature.<br> A 59-year-old woman was seen at the department because of several tumors with sizes of the thumb to little finger in the right axilla. Biopsy of the lymph nodes was performed and histological examination showed a metastatic papillary adenocarcinoma. Clinically, no mass was palpable in the right breast. No signs of abnormality were found in the breast by mammogram, echogram, and xeroradiogram. Systemic examinations also revealed no primary lesion. So the patient was diagnosed as occult breast carcinoma. Right radical mastectomy was performed. Histological examination of the resected specimen showed a 7×8mm papillary adenocarcinoma in the lateral upper quadrant. The t. n. m. classification was t0, n2, m0, in stage III. Adjuvant radiochemotherapy was carried out and she has been doing well postoperatively for 6 years and 6 months without evidence of recurrence.<br> When we encounter a case of metastatic adenocarcinoma in the axillary lymph nodes without any abnormalities in the exploration of the same lateral breast and other organs, this disease can be suspected and, if it is so, the same lateral radical mastectomy should be employed in terms of diagnostic treatment.
