抄録

A 82-year-old woman was seen at our hospital because of abdominal pain and vomiting on December 3, 2005. There was a 5×4cm soft mass with tenderness in the right lower quadrant of abdomen. Abdominal CT scan showed a prolapsed intestine and omentum in the right lower abdominal wall, which was diagnosed as incarcerated hernia of the abdominal wall. Spigelian hernia was suspected, but a possibility of cicatrical hernia could not be ruled out due to her previous histories of appendectomy and oophorectomy. Reduction of the hernia content by hands was impossible, and hence emergency operation was carried out. The hernia sac was found under the aponeurosis of external oblique muscles, perforating the lacertus of transvers abdominal muscles and internal oblique muscles. In conclusion, we diagnosed the case as spigelion hernia and repaired by using Prolene^® hernia system (PHS). Postoperative course was unevenful, and she was discharged very much improved on the 10th hospital day.
