抄録

We reported a 67-year-old right handed woman who had transcortical mixed aphasia after a neck clipping operation of aneurysm at the bifurcation of the left internal carotid and anterior choroidal arteries. She had a moderate right hemiparesis involving the face, with hyperactive tendon reflexes. Tactile and pain sensations were decreased in the right side involving the face without visual field defect. Spontaneous speech was remarkably reduced, and she had a marked distubance in object naming. Her verbal comprehension, reading and writing were severely disturbed. This contrasted with full preservation of repetition of phonemes and short sentences (56 words). A CT scan 3 weeks later showed two infarcts in the left hemisphere, one was in the middle of the frontal gyrus and the other was observed in the left anterior choroidal artery territory. In the CBF (cerebral blood flow) study, ¹²³I-IMP SPECT (single photon emission CT) showed a low flow in the whole left hemisphere except for the left perisylvian speech areas. We concluded that there might be transcortical mixed aphasia caused by disconnection of the perisylvian speech area from the surrounding areas.
