We studied 9 patients (4 men and women; aged 66.7 ± 10.5 years) with cerebral infarction who suffered onset of stroke following antihypertensive therapy : The mean time interval between the onset of stroke and beginning of the therapy was 10.1 days, and in 5 patients cerebral infarction occurred within 7 days. All of the patients had a past history of hypertension. Four patients had diabetes mellitus and one had atrial fibrillation. The stroke was the first attack in all patients except one.<BR>Calcium channel blocker was used in 8 patients, among whom 4 were taking additional antihypertensive agents including β-blocker or ACE inhibitor. In one patient, three different types of antihypertensive agents were given at the beginning of the therapy. In an 83-year-old patient, the maximum dose was administered and the stroke occurred on the following day. The cerebral infarction was most frequently located in the territory of the perforating branches, such as the corona radiata in 6 patients, and also in the cortical branches in 2 patients and in the border zone in one patient. The functional outcome was “wheelchair” in 2 patients.<BR>The starting dose of antihypertensive agents needs to be more carefully decided. A single and smaller dose should be administered at the beginning of the therapy.
