Purpose: To confirm the clinical usefulness of Maddox rod (GULDEN)<br>Methods: Observational case study<br>Result: Case 1:28 years old woman noted vertical and horizontal diplopia with periocular dull pain after subarachnoid hemorrhage. Her visual acuity was 1.2 in both eye. We could not defect eye misalignment using an alternative cover test. Maddox rod test revealed left 4th nerve palsy. An antiosmotic medicine made her diplopia and pain improved. Case 2:28 years old man noted an intermittent diplopia with retrobulbar dull pain. His visual acuity was 1.2 in both eye. We could not defect eye misalignment using an alternative cover test. Maddox rod test revealed a left 3rd nerve palsy. An angiographic examination showed an aneurysm at the basilar artery-posterior cerebral artery bifurcation.<br>Conclusion: Maddox rod is one of the best useful instruments to detect a small amount of eyeball misalignments.
