Purpose: To review two strabismic children without binocularity at the initial visit who were then successful in obtaining binocularity in Fresnel membrane prism therapy before surgical treatment.
Case 1: A 5-year-old girl diagnosed with esotropia in a local clinic at 2 years old, with glasses prescribed and a wait-and-see approach taken. At her first visit at 3 years old to our hospital, her eye position revealed 45 PD base-out for near and distance. Simultaneous perception was negative in binocularity. Fresnel membrane treatment led to her binocularity and decreased the angle of squint.
Case 2: A 9-year-old boy with a wait-and-see approach taken at 2 years old, when his mother took him to a clinic with suspected squint. At the first visit at 9 years old to our hospital, APCT showed 30PD exotropia (XT) with dissociated vertical deviation (DVD). Simultaneous perception was not detected. Fresnel membrane for horizontal correction did not lead to exophoria, but combination of horizontal correction in the dominant eye and vertical correction in the non-dominant eye did. Two months later, simultaneous perception and fusion were demonstrated with anomalous retinal correspondence. In addition, binocular training led to normal retinal correspondence, with the result that the eye position was kept exophoria.
Conclusion: Therapy with Fresnel membrane prisms is so effective for improving the binocular function and reaching good eye position. We conclude that it is very important to perform Fresnel membrane prism therapy before sugery instead of observation without treatment.
