One hundred and ninety-nine cases of fatal bacterial pneumonia have been examined by direct immunofluorescent antibody staining for serogroups 1 to 4 of Legionella pneumophila. Legionnaires' disease was confirmed in 40 cases (20.4%). A total of 41 cases including one other autopsied cases was analysed. The causative orgamisms of 29 cases belonged to serogroup 1. Macroscopically distinct lesions were seen around bronchioles and there was confluent involvement of multiple lobes in most cases. All lobes but most frequently both lower lobes were involved.<br>Microscopic findings showed acute fibrinopurulent bronchopneumonia in which the alveoli were filled with numerous neutrophils, macrophages and abundant fibrin. Lysis of inflammatory cells was frequently observed. Small and blunt pleomorphic intracellular and extracellular bacilli were observed with Warthin-Starry silver impregnation stain. Legionella pneumophila had a double envelope consisting of an inner and outer three layer “unit” membrane shown on electron micrographs.
