Background: We describe cytological and histological findings in a case of mammary hamartoma, an uncommon benign lesion of the breast.<BR>Case: A 30-year-old woman found in a medical checkup to have a mass 2.5 cm in diameter in the right breast was found in fine-needle aspiration cytology to have an epithelial component against a background of scattered bipolar stromal cells. In large epithelial clusters, epithelial and myoepithelial cells cohered to form ductal and acinar structures. Cytological atypia was absent. No fat or smooth muscle cells were observed. Excisional biopsy showed a well-demarcated tumor composed of multiple well-organized epithelial structures similar to terminal-duct lobular units, separated by fibrous and adipose tissue. Adipose tissue was less prominent than the epithelial component and fibrous tissue.<BR>Conclusion: It is difficult to diagnose mammary hamartoma by fine-needle aspiration cytology. The main differential diagnosis is between mammary hamartoma and fibroadenoma. The well-organized epithelial component similar to duct-acinar structures appears to be one of the most important cytological features in diagnosing mammary hamartoma. Fat cells, which are not constantly obtained in aspiration cytology, are not always a clue in differentiating the two lesions.
